受(う)けとってください。

1人(ひとり)4000円(えん)で、家族(かぞく)の世帯(せたい)主(ぬし)にまとめて3月(がつ)末(まつ)までに送(おく)ります。

申請(しんせい)は必要(ひつよう)ありません。
